What to Expect
Jackie and Liz are happy to schedule your first appointment. We will ask for your contact number, date of birth, and insurance information: carrier, toll-free number, ID number, group number, subscriber and patient names and dates of birth. We prefer to verify insurance prior to your appointment. This will help us give you a more accurate estimate of any out-of-pocket costs.
Both Dr. Ray Holt and Dr. Kyra Holt accept PPO plans and are in network with several insurance companies. Contact us and we will be happy to give you more information.
If you have had x-rays in the last 6 to 12 months, you can contact your previous dentist and request they be emailed to firstname.lastname@example.org.
New patients should arrive 15 minutes early to confirm that we have all the information we need to ensure a smooth visit.
Current patients should arrive a few minutes prior to or right at the time of your appointment.
Please be aware that if a patient arrives more than 15 minutes late for a hygiene appointment, your visit may need to be rescheduled. We know you are as busy as we are, so our hygienists maintain a brisk schedule to make sure that you get in and out on time. If you are running late, please call 281.993.9900. We will always try our best to accommodate you, but rescheduling may be necessary.
We accept cash, Visa, Master Card, Discover, American Express, and Care Credit.
Care Credit applications are available upon request. You can also call 800.365.8295 or visit their website www.carecredit.com. Please be aware that your application may take several days before coverage is effective.
